Initial Assessment
An initial assessement usually takes about 1 hour. You will need to have your reading classes (if required), comfortable shoes and clothing to allow you to move freely, your list of medications (if any), your GTN (spray/tablets) and/or inhaler if you have them and a drink (preferably water) or a sugary drink if you are diabetic. The assessment can be conducted at your home or at an agreed venue and will include:

a health questionnaire which will be completed with you. This will allow the programme to be adapted to your own particular needs

information about the programme

information about the exercises and how to use the equipment

explanation of your personal
exercise record sheet, and how to monitor your exercise level and progress

weight and height measurements

short exercise test - nothing more strenuous than walking. This test is only necessary if you have been referred by GP and have either not attended the Phase III Cardiac Rehabilitation Programme, or attended some time ago. If you have completed and been referred from the Phase III Cardiac Rehabilitation Programme, it will not be necessary to complete the exercise test.
The Assessment will cost £10 and is payable at the time of the appointment. A cheque or cash is acceptable.
A short summary of the Assessment will be sent to your GP.
